SEPTIC SYSTEM ABANDONMENT PERMIT APPLICATION INSTRUCTIONS

1. Fill in ALL information on page one of Application Form DH 4015 concerning owner, agent and mailing address. This means DON'T LEAVE ANY BLANKS in these sections.

2. Fill in the street address and provide directions on how to get there.

3. Attach an Agent Authorization Form signed by the applicant, if you are an agent for the applicant and are not a registered septic system contractor, State of Florida certified plumber or State of Florida licensed building contractor.

4. A fee for a Septic System Abandonment Permit must be paid. The permit will be valid for 90 days after your connection to a sewer system.

5. Following completion of the abandonment, an inspection is required. Please call this department for an inspection.

6. A receipt for pump-out of the septic tank must be provided for final approval of the abandonment.
