NEW SEPTIC SYSTEM CONSTRUCTION
PERMIT APPLICATION INSTRUCTIONS

1. Fill in ALL information on page one of the Application form
DH 4015. This means DON'T LEAVE ANY BLANKS. If your information is incomplete or found to be inaccurate, your application will not be processed.

PROPERTY INFORMATION concerning your lot, block, subdivision or metes and bounds description, property I.D. number and property size in acres or portion of an acre may be found on your deed, survey, tax bill or from the Volusia County Tax Assessor's office or Clerk of the Circuit Court. Information concerning your recorded subdivision date may be obtained from the Volusia County Building Department, 423-3300, extension 2272; information concerning your unrecorded subdivision date may be obtained from the Building Department, 423-3300, extension 2288.

BUILDING INFORMATION will include each living unit and/or building that is served by the subject septic system, number of bedrooms (if applicable), building area (means heated and cooled area) as measured from exterior walls, number of persons and business activity (if applicable).

2. Attach two copies of a site plan. The site plan and all features shown on it MUST BE TO SCALE, and must show lot dimensions, the location of the proposed septic system on the lot, the proposed building location, driveways and sidewalks, surface water bodies, stormwater drainage features, potable and non-potable wells. Surface water bodies, stormwater drainage features, potable and non-potable wells, buildings and septic systems on neighboring properties within 75' of your lot lines must also be shown. If neighboring properties are vacant, write "vacant land" where applicable. The site plan must be signed by the applicant or authorized agent.

The site must be prepared for evaluation. It must be accessible. Lot boundaries must be clearly marked. The proposed septic system area must be accessible and must be clearly marked. If the lot is not located on a paved road, mark a tree, fence post, utility pole or other permanent feature close to the proposed septic system area so that we may use it for placement of our benchmark. We have marking tape available for this purpose. If the area for the proposed septic system has not been scraped or filled, do not scrape or fill prior to evaluation of the site.

If the inspector is unable to perform the site evaluation due to inaccessibility or lack of proper site marking, a reinspection fee will be required and collected prior to the inspector's re-visiting the property.

The benchmark set by the inspector is a permanent mark. Relocating or removing the benchmark without prior notification to the Department may render your construction permit VOID, and cause delay of the project and additional expense.

3. Attach a floor plan. The floor plan must clearly show all outside wall dimensions and the room configuration within the home or building.

4. Attach an Agent Authorization Form signed by the applicant, if you are an agent for the applicant and are not a registered septic contractor, State of Florida certified plumber or State of Florida licensed building contractor.

5. The permit will be valid for 18 months after the date of issue.

FOLLOWING SEPTIC SYSTEM CONSTRUCTION

Following septic system construction, there may be some requirements that have not yet been met. These requirements may include installation of wells or water lines, sidewalks, driveways, or roof gutters. Per Florida Administrative Code, if an inspector must return to verify compliance with these requirements, a re-inspection fee must be paid prior to each re-inspection. Please consult with your septic system contractor concerning timing of the construction inspection so as to avoid or reduce the number of re-inspections you may need to gain final approval of the installation.
