EXISTING SEPTIC SYSTEM APPLICATION INSTRUCTIONS

1. Fill in ALL information on of the Application form DH 4015 Page One and on DH 4015 Page Four. This means DON'T LEAVE ANY BLANKS. If your information is incomplete or found to be inaccurate, your application will not be processed.

PROPERTY INFORMATION concerning your lot, block, subdivision or metes and bounds description, property I.D. number and property size in acres or portion of an acre may be found on your deed, survey, tax bill or from the Volusia County Tax Assessor's office or Clerk of the Circuit Court. Information concerning your recorded subdivision date may be obtained from the Volusia County Building Department, 423-3300, extension 2272 or by going to this link: http://www.clerk.org/or_plats/publicrecords/publicrecords.jsp
Information concerning your unrecorded subdivision date may be obtained from the Building Department, 423-3300, extension 2288.

BUILDING INFORMATION will include each living unit and/or building that is served by the subject septic system, number of bedrooms existing and proposed (if applicable), building area existing and proposed (means heated and cooled area) as measured from exterior walls, number of persons existing and proposed and business activity (if applicable).

2. Attach a site plan. The site plan and all features shown on it MUST BE TO SCALE, and must show lot dimensions, the location of the existing septic system on the lot, the existing and proposed building location, driveways and sidewalks, surface water bodies, stormwater drainage features, potable and non-potable wells. Surface water bodies, stormwater drainage features, potable and nonpotable wells on neighboring properties must also be shown. If neighboring properties are vacant, write "vacant land" where applicable. The site plan must be signed by the applicant or authorized agent. The existing septic system area must be accessible and must be clearly marked.

3. Attach a floor plan. The floor plan must clearly show all existing and proposed outside wall dimensions, the existing and proposed room configuration within the home or building, and any other features that will be added or changed.

4. Attach the Existing System and System Repair Information form. The exact year of original septic system construction is mandatory. Give as much of the rest of the requested information as you can.

5. Following an inspection of your property by our department, you may be asked to submit a receipt for pumpout of your septic tank.

6. Attach an Agent Authorization Form signed by the applicant, if you are an agent for the applicant and you are not a registered septic contractor, licensed plumber or licensed builder.

7. A fee for an Existing Septic System Application must be paid.
