By Mail:
Check or money order payable to Volusia County Health Department. Visa, MasterCard and American
Express are also accepted. Credit card number and expiration dates are required. Requests
sent by mail must have the applicant's signature. Orders will be returned unprocessed if the
requests are incomplete.
In Person:
Checks, cash, money orders, Visa, MasterCard or American Express. Current valid ID is required
in person. Acceptable forms of ID are Driver's License, State ID Card issued by Department of
Motor Vehicles, Military ID or Passport.
