Mission of the Health Promotion and Education Program
To improve individual and community health by preventing and reducing the impact of chronic disease through education and awareness of the risk factors, signs and symptoms related to chronic disease and in developing environmental and social policy strategies that support healthy life choices.

Floridians inspired to be healthy and free of preventable chronic diseases and disabling conditions living in supportive communities.

Burden of Chronic Disease
In 2001, chronic diseases accounted for six of the top ten leading causes of death in Florida. Of the 167,181 deaths that occurred in Florida, 71.2% were due to chronic disease. Chronic diseases are characterized by having multiple risk factors, a long period between onset and development of symptoms, a prolonged course of illness, functional impairment or disability, and not being curable.

Population
The target population is the entire community. However, special emphasis may be placed on specific populations disproportionately affected and least likely to receive the proposed interventions, i.e., the uninsured, low-income and/or minority populations. Specially targeted activities may focus on, but are not limited to: neighborhoods, census tracts, attendance at educational facilities, zip codes or counties. Intervention activities shall be selected based on data, need, ability to impact and demographic characteristics of the county and any specific target populations.

Focus on Policies and Environmental Change Strategies
Many of the state and federal public health agencies are now focusing on policy and environmental change strategies. Research is currently being conducted and initial results indicated that establishing health related policies and environmental changes impact many individuals simultaneously by providing a foundation for making or maintaining healthy life choices. This new focus combined with individual education and awareness should provide the optimal situation for a healthy lifestyle.

Community Health Promotion program:
Includes all of the following:
  • community participation with representation from at least three of the following community sectors: government, education, business, faith organizations, health care, media, voluntary agencies, and the public
  • community assessment, guided by a community assessment and planning model, to determine community health problems, resources, perceptions, and priorities for action
  • targeted and measurable objectives to address at least one of the following: health outcomes, risk factors, public awareness, services, and protection
  • comprehensive, multifaceted, culturally relevant interventions that have multiple targets for change
  • monitoring and evaluation processes to determine whether the objectives are reached

Community Indicators:
What are the Indicators?
"Community Indicators" are measuring systems, designed, developed and researched by community members to assist them in steering their communities on a collectively desired course. Citizen groups across the country are collecting data and developing indicator reports to educate the public about important issues or values, like environmental quality, economic vitality, and community health. In technical terms, indicators are presentations of data that show changes and trends over time; they are most often illustrated as charts and graphs, because these images allow people to see the trends at a glance. A well-researched indicator report provides community members, leaders and policy makers with the information necessary to make decisions that improve the quality of life over time.
