The Health Planning Council is a 501(c) 3 and is one of 11 local health councils created by Florida Statute Chapter 408.33 to provide coordinated health planning services designed to enhance the provision of accessible, affordable, and high quality health care services to all persons residing in Baker, Clay, Duval, Flagler, Nassau, St. Johns, and Volusia Counties.

With offices located in both Duval and Volusia County, the Health Planning Council provides preventive health care initiatives such as emergency room divergent programs for uninsured diabetics, securing free immunizations for the uninsured, the HIV/STD mobile van outreach initiative, and managing Ryan White coordinated patient services and Housing Opportunities for Persons with AIDS in Volusia and Flagler counties.

The Health Planning Council is also a steward of coalition building and convening communities around critical health care issues. Our value to the community comes in the form of non-partisan services, trusted advocacy, and a focus on disease prevention and management. As a respected neutral community partner and non-profit, we have the ability to build health care capacity, empower communities, and fill service gaps where traditional governmental programs can not.
